Financial Assistance Choice
The SW Rotary Club of Wichita Falls has two separate funding request forms to service difference sized request.  Those request under $1000 may use the SW Rotary "Short Form", while those request for $1,000 or more in financial assistance should use the financial assistance "long form."  Please Click on the appropriate link below to use the appropriate form for your funding request.
